STEP-BY-STEP PROCESS FOR AVAILING WELLNESS LEAVE (WL)

A. For Non-Teaching and Related-Teaching Personnel

  1. Accomplish the Leave Form

    • Fill out CSC Form No. 6 in two (2) original copies.

    • File it the same way as Vacation Leave, Sick Leave, or other leave privileges.

    • Until the revised CSC Form 6 is issued:

      • Indicate the leave under “Others” in Item 6.A

      • Specify: “Wellness Leave (CSC MC No. 01, s. 2026)”

  2. Observe Filing Timeline

    • File the application at least five (5) days before the intended leave.

    • In emergency cases, file the application immediately upon return to work.

  3. Submit to Immediate Supervisor

    • The supervisor reviews and endorses the application.

  4. Processing and Approval

    • The application is processed and approved following the Revised Signing Authorities under DepEd Order No. 001, s. 2023 (Annex C) or its latest amendment.

  5. Avail the Wellness Leave

    • You may take:

      • Up to five (5) days per year

      • Either consecutively (maximum of 3 days at a time) or on non-consecutive days

B. For Teaching Personnel

  1. Accomplish the Leave Form

    • Fill out CSC Form No. 6 in two (2) original copies.

    • Until the revised CSC Form 6 is issued:

      • Indicate under “Others” in Item 6.A

      • Write: “Wellness Leave (CSC MC No. 01, s. 2026)”

  2. Submit to School Head

    • File the application at least five (5 days before) the intended leave.

    • In emergency cases, file immediately upon return.

    • The School Head reviews and recommends approval.

  3. Endorsement to the ASDS

    • The School Head forwards the application to the Assistant Schools Division Superintendent (ASDS) for approval.

  4. HRMO Processing

    • The Human Resource Management Officer (HRMO) processes the request and fills out Item 7.A (Certification of Leave Credits), reflecting:

      • i. Total WL credits to date

      • ii. WL credits to be used

      • iii. Remaining WL credits after this application

  5. Approval and Recording

    • Once approved, HRMO records and updates the teacher regularly on their Wellness Leave balance.

  6. Service Continuity

    • The School Head assigns substitute/relieving teachers.

    • Relieving teachers are entitled to teaching overload pay per DO 005, s. 2024 (or future amendments).

C. For Contract of Service (COS) and Job Order (JO) Personnel

  1. Check Contract Provision

    • Ensure that Wellness Leave is stated in your individual contract or relevant issuance.

  2. Accomplish the Prescribed Form

    • File the WL application using the prescribed form (Annex A).

    • Follow the same procedure as Vacation Leave and Sick Leave for regular/permanent non-teaching personnel.

  3. Observe Filing Timeline

    • File at least five (5) days before the intended leave.

    • In emergency cases, file immediately upon return to work.

  4. Processing and Approval

    • Applications are processed and approved following the Revised Signing Authorities under DO 001, s. 2023 (Annex C) or its amendments.

  5. Salary Treatment

    • No salary deduction during Wellness Leave.

    • COS/JO personnel continue to receive full pay for the days they use WL.
