The Department of Education has already adjusted the school forms to be used for this School Year 2021-2022 as a response to the existing pandemic which required the country to use distance learning as its modality.
The said ADJUSTED FORMS are now available for download from the Learning Information System (LIS) website (www.lis.deped.gov.ph).
Class Advisers may now log in to their respective accounts (www.lis.deped.gov.ph), proceed to "School Forms" tab on the upper part of the account then select the class section.
From there, you will be able to download the forms such as School Form 1, School Form 2, and School Form 3.
Teachers are discouraged to make their own adjustments to the said forms as they are already made available in the system for download.
ADJUSTED SCHOOL FORMS are now AVAILABLE FOR DOWNLOAD in the LIS
